BACKGROUND: The City of Lawton provides vision coverage to city of Lawton employees and their families. The City currently offers vision care through Blue Cross Blue Shield and Primary Vision Care Services. Primary Vision Care Services, Inc. offers vision coverage at the following monthly rates; $10.75 for employee only, $19.75 for employee and one dependent (spouse or child), $28.00 per month for the employee, spouse and child/children. This is a renewal of the contract signed in 2023 and does not include any rate increases or decreases. This product is 100% employee paid.
